We encourage you and your child(ren) to stop in for a tour of Kidz Clubhouse. One of our highly trained staff members will show you around our licensed facility, go over our parent handbook, and help you fill out the required forms. Registration only takes about ten minutes.

In order for your child to attend Kidz Clubhouse, the following forms must be on file:

• Birth to 29 months requires a physical examination performed within 3 months prior to enrollment with restrictions noted and updated annually.
  
• 30 months to 5 years requires a physical examination performed within the preceding year and updated every 2 years.
  
• School Age: A signed statement from the parent indicating that the child is in good health with activity restrictions noted.
  
  Other required forms include family information, allergies, emergency contacts, field trip and picture permission, and health forms. There is a $125 initial registration fee per family.
